HIV and STD education for all

Now more than ever it is important to explore the strategies for prevention of STD's with the AIDS pandemic, a patient with and STD may have acquired HIV infection or may get HIV infection in the future. Therefore, treatment alone is not sufficient and health education to prevent sexually transmitted diseases is of paramount importance. This will entail discussion on sex and sexuality from doctor to patients, from parent to child and from teacher to students.

Communicating about STD's is extremely difficult as it is necessary to discuss sexual practices, a topic may people in our culture rather be left alone, and bring communication to a very personnel less to be effective. In order to prevent STD and the spread of HIV infection, it is mandated for individuals to change entail choosing not to have many partners or as to use condoms.

SOCIAL ASPECTS
STD's are with us for long. The risk factor for contracting a STD is having many sexual partners, or to have a partner who has many sexual partners. Although anyone can get a STD, certain groups of individuals are more prone due to their life style and social or economic and environmental circumstances. This may include:

Migrant workers who live away from their families;
Men who travel as a part of their job;
Those who resort to sell sex for financial reasons;
Armed force personnel who are posted away from their families.
The clients of CSW coming from the society from among about 20 categories of population of reproductive age can be mapped as follows.
The CSW themselves women and virgins & children trafficked.

ROUTE & RANGE OF TRANMISSION OF STD/HIV/AIDS IN BANGLADESH
CSW at located places
CSW at Hotel & Residential place, Rest houses, Bungalow etc., floating on Road side/High ways etc.
Clients- Identifiable : Truckers, Drivers, Long Distant workers etc.
Unidentifiable : A Numbers of population from about 20 categories of the society.
Wives and other : Infected by the husbands or irregular partner irregular partners who happen to receive transmission
Children : Infected by mothers during pregnancy, after pregnancy during breast feeding.

A good number of such women & children are vulnerable due to being infected without being a party to the sex working and in addressing them. The whole population need be covered in message giving.

The clients receiving infection from CSW or other irregular partners transmit it to the wives & from wives to children. This vicious circle is causing infection on daily basis until they are not equipped with prevention practices.
Beyond the CSW at located places, a large number CSW disguised in sex trade are also living around the society with the risk behavior and vulnerability. This group can hardly be identified. So, mass thrust of message giving is essential for which the OSDUY is distributing AIDS-Okal Mrityu. The infected wives and children have to bear the risk to STD and HIV both without a party to the sex net working. Absistance or a mutually faithful relationship with one life long partner are very effective in preventing STD and AIDS. For health education approaches to work grow addiction to sex networking and the approach must be more practical. There is proven evidence that those who have STD will not easily change behavior and they shall have to be convinced to have the condom and to seek proper treatment for their STD's. In an era of HIV/AIDS practical messages and approaches are essential. If everybody from everywhere is not self-prevented and self guarded, it may not be possible to prevent anywhere and it will continue to spread endlessly. So practical message on STD/HIV and AIDS should be brought home to everyone of reproductive age and everyone shall have to be convinced that it is in their own interest that they should follow the safe practices to save himself, his wife and to protect the child and everyone should be in search of a decent and STD free reproductive track body.


If we are really aware of our job, our task will be to communicate the practical messages of STD/AIDS prevention for self-prevention of them for ends of nationwide safety. A thrust of message therapy should be delivered at every nook and corner of the society with counseling & feed-back so as to make the whole nation stand safe guarded against STD and AIDS and to prove that man is not powerless against these pandemic.

The main problem is this that the government departments of medical services are inadequate to meet the needs of the time in the context of effective treatment, care and control of STD/HIV/AIDS. About 4 million people including adolescents, adults and people of reproductive age are infected with STD and the number is increasing by days as there is no vision or mission of any agency to help ensure effective and complete treatment.

That the reproductive track requires being ill free for the upcoming generation is mandatory for any society. In the context, a policy on no further infection of STD and clean reproductive track is to attained for a next generation. But the people attached are neither aware of the effective treatment nor any agency remain active for the achieve of this.

It is a continuous process of education information and communication to all of everywhere.

HIV and ICT Development

40 million youth in Bangladesh are to be addressed under the employment exchange proposition. There is no specific mission and vision to accommodate the whole group of youth in a thrust. By the by, the number of youth shall have to be developed into resource persons through training and motivation works.

The community through posting of ICT center at each upazilla (Sub-district) by end of 2004 and at unions by 2008 and at villages by 2010, I think.

All these group of youth shall also be equipped with the mission and vision of sustainable livelihood and subsistence earning on a self-help manner through exploration of homestead capital development.

The already publication of AIDS has been engrossing upon sexual & reproductive track management aspect to the community through participation of "Social Group" and the community. The content of self-prevention for infection of HIV/AIDS/STD and thereby a great number of youth and people are on track effective information of sexual health management.

Human rights and HIV/AIDS

Sex work come under the purview of human rights whenever it is a case of extra marital sex or commercial sex other than the sacred sex work between the couple. Sex between a couple naturally does not come in question on HIV/AIDS infection unless anyone has undergone extra marital sex with anyone and with no protective measures against infection. Sex limited within couple is a sacred task blessed by god and it becomes unblessed when it goes beyond and it has now the burning issues that unprotected extra marital sex is the root of the evil of HIV/STD/AIDS. The persons not yet married but undergoing sex with many and does not care for safety practices are absolutely in high risk situation. But if he can follow a safety practices in case of multiple sex work, he may happen not to be in high risk. Because many undergoing multiple sex with cautions and prevention and avoid contact of body fluid, blood, saliva, vaginal fluid etc. can defend himself from the infection.

STD is the only indicator of unsafe sex. Besides, std has a three fold linkage with HIV/AIDS for which syphilis patients bear ten fold risk of acquiring HIV/AIDS and Gonorrhea patients bear four fold risk more than others. Besides, the majority people suffering from STD do not bother for effective treatment of STD and remain infected lifelong and continue to infect others. That is why there has been no end to this infection of STD and by far the HIV/AIDS. STD and HIV/AIDS are the result of individual ignorance of safety and if everyone from every where could be made to realize safety sexual behavior with other precaution the infection could have been averted more early. But inspite of documented successful interventions in many countries the rate of infections may have slowed down but not have stopped anywhere.

It is now the world thinkers to go to a step for no further infections which require dedication and cool calculation to the program management about how to bring all of every where into the same no how of safety from others infections. It is almost easier to be saved but many are still acquiring infections inspite of sides the hue and cry against HIV/AIDS. This is the individual tragedy of ignorance and irresponsibility and acting against civilization. The people already infected have acted against themselves and infecting others without their knowledge or notice of those who are being infected and every stage may be said to have over heard the voices of human rights. As to the extramarital Sex and Human rights, The word of commercial sex is a center of breach of human rights.

Young girls and young women are the mainstream people of sex world who are subjected to abuse of human rights at all stages of their activities from the point of time of entry into the sex world.

The poverty stricken illiterate, half-illiterate young girls & women of the rural and urban areas are made to flock to this task by the underground ill motivated people of the locality. These women are trapped in this job willingly or unwillingly with a dream of luxurious life told before hand. As soon as they get in the trade they realize that they have been trapped and begin to suffer in all stage of their activities and the principal share of money they earn are eaten by the middleman, house owner, terrorists, polices, and the goondas. They are totally lost in oblivion and the power to defend from infection is also lost. And this is why the rate of infection could not yet be controlled or minimized.

The extramarital sex world need be intervened under the international guidelines on HIV/AIDS in the context of human rights 1-12, these guidelines are drafted at second international consultation on HIV/AIDS.
